Appointments
Our office schedules visits by appointment only. Walk-ins are not permitted. We will make every effort possible to see emergent and urgent conditions in a timely manner. Every effort is made to see our scheduled patients on time.
Patient Forms
The patients coming to our office for an appointment can download the patient history form and print at home. Filling out these forms ahead of time will save a significant amount of time during your visit. Please be prepared to bring this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your appointment.
Cancellations
If you are unable to keep your scheduled appointment, please call us at least 24 hours in advance. Also, please call if you know you are going to be late. This courtesy allows us to accommodate other patients.
New Patients
On your first visit to our office, please plan to:
- Bring a list of all medications you are presently taking and a detailed list of all previous
surgery procedures. - Bring any imaging studies (x-ray, CT-scans, and MRI’s). Be sure to bring a CD of your images, not just the written report.
- Bring your insurance card and a photo I.D.
Referrals
If your insurance requires you to have a referral, it is your responsibility to obtain the referral and make sure it is in our office prior to your appointment. If we do not have your referral at the time of your visit, your appointment will be rescheduled.
Payment Policy
Co-payments are collected at the time of visit. For your convenience, we accept cash, checks, debit and credit cards (Visa, MasterCard, Amex, and Discover). If you come without your co-payment your appointment may be rescheduled.
If you do not have health coverage, or are in financial distress, a payment plan can be established. When on the payment plan and a situation arises that affects your ability to make your payment, please contact our Business Office immediately at 401-738-7442. Communication is very important. If your account is placed with our collection agency, you will be unable to make a future appointment until your debt is resolved.
A fee of $25.00 will be added to your account for returned checks.
Minor Patients
If your child is under 18 years of age, he or she must be accompanied by a responsible adult at the time of the appointment. Without a responsible adult with the underage patient, the appointment will be rescheduled. It is unlawful to treat a minor without the consent of a parent or legal guardian.
Patient Termination
Dr. Mirrer reserves the right to discontinue care and discharge a non-compliant patient at any time. The patient will be provided with the names and phone numbers of other local orthopedic surgeons in the area. Our doctors will continue to provide the patient with emergency care for 30 days after the termination.
